Both lower blood pressure by interrupting the same hormone system at different points. Lisinopril is an ACE inhibitor: it blocks the enzyme that converts angiotensin I into angiotensin II, so less of the vessel-tightening hormone is made. Losartan is an angiotensin receptor blocker: the hormone is still made but cannot act on its receptor. That difference explains the best-known side effect split, because ACE also breaks down bradykinin, and the bradykinin that accumulates under lisinopril causes the persistent dry cough that losartan does not. Losartan is much the heavier molecule at 422.9 against 405.5 and carries a tetrazole ring. Both are prescription medicines.
